LS observed a staffing pattern posted dated for January of 2024, and did not include names of either teacher in charge today.
LS observed two teachers unknown to the Office of Childcare.
Per the Area Director, on the day of the incident, the child was able to leave the carpet, walk around the table that was in front of the double doors, open and walk out of the double doors, walk through the hall, out the front door and into the parking lot un-noticed. LS discussed the importance of ensuring children are safe and secure at all times. LS advised that no child should be subjected to neglect at any time.
LS inquired about the center's policy for ensuring all children are accounted for at all times. LS interviewed the teacher in charge on today's date, who was not present on the day of the incident. That teacher stated that children are watched closely while in the cafeteria and that roll call is done at least once per hour. LS observed Both teachers supervising children closely on today's date. Per the Area Director, on the day of the incident, the child was able to leave the carpet, walk around the table that was in front of the double doors, open and walk out of the double doors, walk through the hall, out the front door and into the parking lot un-noticed. LS discussed the importance of ensuring children are safe and secure at all times.
LS conducted a complaint investigation on today's date. Complaint is regarding a report from Director that a child ran out of the program, but that the child was found shortly after, brought back to the cafeteria, and the child's parents were called. LS interview the Area Director about the incident over the phone. Area Director stated that according to the Teacher who was in charge that day, the child was playing on the carpet near the side double doors, to the right of the stage, and left the cafeteria without being seen. Area Director observed hallway camera footage which showed that the child was missing for approximately 3 minutes from the time that the child left the cafeteria to the time that he was found in the parking lot. LS discussed the importance of children being supervised at all times.
LS observed evidence of rust present on the bathroom stall accessible to the boys in the program. LS observed evidence of one sink in the girls' bathroom with a bucket located under the pumping and no water accessible to the girls in the program.
LS observed no evidence of a medical for one staff member.
