At the time of this inspection the provider is missing a whistle from the first aid kit. The provider is to obtain this missing item and submit evidence to the LS.
At the time of this inspection the provider has a large bin full of first aid kit items. The provider is to place the required items into a bag/case to ensure these items are able to be transported easily when on an activity away from the home.
At the time of this inspection the provider has a child whom medical documentation states the need for an EpiPen and albuterol. However, the provider does not have proper evidence of completed authorization forms. The provider has stated the mom indicated it is no longer required however, the child's medical paperwork from the doctor still indicates the need for such medication.
At the time of this inspection the provider has Budesonide present for a child enrolled in care, however, the prescription is missing the label from the pharmacy. The provider has stated the mom indicated it is no longer required however, the child's medical paperwork from the doctor still indicates the need for such medication.
The provider has an Auvi-Q and Budesonide on site for a child enrolled and present in care that is expired. The provider has stated the mom indicated it is no longer required however, the child's medical paperwork from the doctor still indicates the need for such medication.
At the time of this inspection the provider has a health assessment (part 1 and part 2) along with immunizations that are missing from a child's file. The provider is to obtain these missing documents and submit evidence to the LS. The provider stated this is the child's first day however, the LS did not observe evidence of an appointment to have the forms completed.
