Of the files reviewed, one was missing health inventory part 1 and two were missing the parent signature and date. Director will send these to LS and review all files to ensure they are complete.
Of the files reviewed, one was missing a health inventory part 2. Director will send this to LS and review all files to ensure they all are complete.
Of the files reviewed, two were missing lead tests. Director will send these to LS and a statement that she has reviewed all files and all have lead tests as needed.
Several rooms had outdated staffing patterns posted- B, D, I and H. Director will send new ones for these rooms and a statement that all posted staffing patterns have been reviewed and are up to date.
When LS arrived, two classrooms had aides and no teacher present. Staffing patterns and a statement of correction will be sent.
When LS arrived, there were two aides in Room B, toddlers, and one aide in Room H, school age. Director will send staffing patterns for these rooms showing adequate teacher coverage for these rooms, especially in the mornings.
At the time of the inspection the center was in compliance, however there were several staff that reported that there has been a teacher who left an aide alone in a classroom with children while they are awake.

One medication form expired in 2023 and needs to be updated. Director will get this and send to LS when received by child's parent.
Three staff need a new medical. Director will send these to LS when obtained.
Four teachers did not complete their annual training for the last full year. Two have already made this up and director will send this to LS, plus the missing training for the other two teachers.
