Three staff medicals were missing confirmation from the physician that the TB test or screening was completed.

LS observed 3 children's medication forms that were not complete.
Room 105, and 106 refrigerator thermometers were registering over 40 degrees which is not sufficient for protecting perishable items from spoiling.
The refrigerators in Rooms 105 and 106 were registering at temperatures above 40 degrees.
LS interviewed staff. Two staff had concerns about a teacher's routine for helping children go down for nap. When expressing the concern there was a follow up discussion with administration. Administration states that the concern was handled within the facility and they found no need to escalate the information to additional agencies. Facility is reminded that any time there is a concern about abuse or neglect the report must be to the protective services department of the local department of social services or law enforcement.

At time of the inspection, a new employee was observing in a classroom without being cleared by the OCC. This was discussed with the three Directors and personnel forms were given to the LS at inspection.
