Licensing Specialist observed two classrooms that are missing baseboard molding. Licensing Specialist observed a storage cabinet in room 3 that does not lock properly. Please submit photographic evidence identifying how this has been corrected.
Licensing Specialist observed a hose that is unraveled and a piece of yellow tubing on the playground, a wooden railroad tie that is rotten and broken at one end, two broken orange cones, several missing fence posts along the back side of the playground, rusted chains on the swings, blackish/green substance on the climbing equipment, and wild mushrooms growing on the playground. Please send Licensing Specialist photographic evidence indicating how these items have been corrected.

This mandatory review inspection recorded no violations or advisories.
Specialist observed staff employed that the facility failed to properly notify OCC about.
Specialist observed several medicals more than five years old and two with no medicals at all.
Specialist observed five staff missing basic health and safety that has been employed longer than 90 days
