LS observed staffing patterns posted that did not have the correct staff in the classrooms. Facility to submit new staffing patterns for all classrooms in the facility.
LS observed an aide alone in a classroom of 2's with 6 children present. Assistant Director went into the classroom to correct at the time of inspection. Facility to submit staffing pattern showing qualified staff in each classroom. Aide in the classroom has been cleared through MSDE.
LS observed a qualified aide only in charge of 6 two year old children. Assistant Director entered the classroom for the day in order to meet the regulation. Corrected at the time of inspection. Facility to submit a staffing pattern showing qualified staff in the classroom at all times.
LS was not notified of the start of a new hire within 5 days. Facility to submit a statement of understanding that within 5 working days of its occurrence, provide written notification to the OCC of a new hire and all of the required forms and information.
Facility did not provide the OCC documentation within 15 days after adding an employee to meet the requirements of this chapter for the assignment, unless documentation already is on file in the office. Facility to submit a written memorandum of understanding stating that the regulation will be followed.
LS noticed a staff member that is on the staffing pattern that had not been cleared by OCC. Facility to submit a written memorandum of understanding stating that the staff may not be in the classroom with children until CBC's have been cleared.
