This mandatory review inspection recorded no violations or advisories.
LS interviewed the director and a teacher in room 11 in reference to the aide blocking a 3 year old child in the bathroom. The teacher stated that she was lining children up to go outside and noticed that one child was missing. She then heard a child crying and observed the aide in room # 11 sitting in a chair that was backed up against the bathroom. The teacher stated that she asked the aide if the child was in the bathroom and the aide laughed and said that she put the child in the bathroom to punish him because he was being bad. The teacher told the aide to move and saw that the child referenced in the incident was inside of the bathroom with the lights out. The teacher stated that she told the aide that she can't block kids in the bathroom when they are being bad because that is neglect, and the aide laughed at her again. The teacher then went to tell the director what happened, and the aide was immediately removed from the room and suspended. The aide has also been terminated, and has not been back at the center since 05/12/2025. An employee may not subject a child to abuse, neglect, mental injury, or injurious treatment.

The operator shall ensure the safety and security of each child at all times.
LS conducted a complaint investigation on today's date. LS interviewed the director and a teacher in room 11 in reference to the aide blocking a 3 year old child in the bathroom. The teacher stated that she was lining children up to go outside and noticed that one child was missing. She then heard a child crying and observed the aide in room # 11 sitting in a chair that was backed up against the bathroom. The teacher stated that she asked the aide if the child was in the bathroom and the aide laughed and said that she put the child in the bathroom to punish him because he was being bad. The teacher told the aide to move and saw that the child referenced in the incident was inside of the bathroom with the lights out. The teacher stated that she told the aide that she can't block kids in the bathroom when they are being bad because that is neglect, and the aide laughed at her again. The teacher then went to tell the director what happened, and the aide was immediately removed from the room and suspended. An operator shall ensure that each child receives adequate supervision and care at all times which is appropriate to the individual needs, age and capabilities of the child.
LS did not observe a qualified teacher in rooms 2, 4, 6, 10, 11 or 12.
LS observed two staff members with 8 toddlers in room # 6.
LS observed 26 children in room #13 with 2 staff members. 12 of the 26 children were preschool 4 and 5 year olds, and 14 of those children were school age.
LS observed seven children needing updated emergency forms.
LS observed two children needing part 1 of the Health Assessment and two children needing part 2 of the Health Assessment.
LS observed three children needing a lead screening.
LS observed one child needing vaccines.
LS did not receive continuing training for all aides, teachers, and director on staff for the last cycle. Training for last cycle and this cycle are pending.
LS did not observe a qualified teacher in room # 6. LS did observe an approved substitute in room # 6, who informed LS that she is the assigned teacher in that room.
