The LS observed that the Provider needed her licenses number added to her sign. The Provider's daughter added the number to the sign correcting this non-compliance on site.
The Provider has a handbook, however all the files don't have documentation that they received the handbook.

The LS observed that the Provider's emergency forms needed information added: CB, U, S, AA, SM, KC.
The Provider needs to add Physician's info. to SM's emergency form, enrollment dates and hours of attendance to C.B. AA, and K.C. The following children need lead testing: C.B, AA, SM, KC.
The LS observed that there is another room in the basement that the Provider was using for diapering and nap. The LS had the Provider remove child care items and this room will be added and restricted to the Provider's licenses.
The LS observed that there is no gate at the Provider's stairs. She has a protective barrier between the foyer and restricted rooms.
The LS observed that under the Provider's sink is locked using a rubber band type band. The LS advised the Provider to get the correct type of child-proofed lock and place it under the sink. The Provider will send the LS a picture of the correction.
The LS observed that some of the toys, had standing water. The Provider's daughter poured it out to correct this non-compliance.
The Provider has an emergency plan. She needs to review it and complete. To correct this non-compliance the Provider needs to email the LS a completed copy of the emergency preparedness plan.
The Provider needs to add a triangular bandage, whistle, and fragrance free soap.
The LS observed that there was a razor laying on the shower floor of the bathroom the children use. The Provider removed the razor out of reach, correcting this non-compliance on site.
LS did not observe continued training.
The LS observed that siblings shared an emergency form, the provider will have the parent complete a separate form and will email the LS the correction.
The LS observed that the sliding board is broken there is a nail sticking up and a nail missing. The provider will fix this as soon as possible and will send pictures to the LS.
