The LS reviewed the Provider's eight emergency forms and observed that that four are missing some information such as Physician's info, emergency pick-ups address, enrollment date and hours/days of attendance.
The Provider just got a new AC unit and needs a cover or a barrier. The Provider's sub. ordered a cover during the inspection and will send a picture of the AC unit covered to correct the non-compliance.

The Provider hasn't worked on continuing training hours.
The LS observed that the Provider answered the door for the LS; the Provider left two children in the basement area unsupervised. The LS and the Provider discussed Active Supervision and the LS gave the Provider a visual to post as a visual cue.

LS observed 4 children's files missing a lead screening or test results. Provider will request documents from parents then forward a copy to LS.
LS did not observe updated dates on the emergency form of 2 children in care. Provider will have the parents date the emergency forms then send a copy to LS.
LS observed 3 incomplete emergency forms, 2 emergency forms that need to be updated; 1 file that needs a health inventory I and II; 2 files that need lead screening/test; 2 files that need completed medication administration documents , asthma and allergy action plans.
LS did not observe medication or medication administration/action plan forms for children with asthma and allergies.
LS did not observe 1% milk for children 2 years old and older. Provider will submit a picture of 1% milk.
The Provider's outside area is under construction; upon completion of the outside area the Provider will email the LS pictures. The Provider walks the children to the neighborhood playground.
The LS observed that the Physician's information was not completed on the emergency form. The provider will get this filled out and will email a picture to the LS.
The outdoor area has ample space; during the inspection the LS observed that the outside area had old diapers that needed to be placed into a trash bag. The LS observed a broken rake, broken pots and the AC wasn't covered. The walkway to get to the outside area was filled with clothes, decorations and other items that needed to be stored.
The LS observed that the laundry room, that the children walk through needs to be cleared out so it gives a clear walkway for the children. The LS observed a jack, decorations, lots of clothing, detergent etc.
