Licensing Specialist observed the towel holder unsecured above the toilet with the screws coming out of the wall.
Please remove items in disrepair and water hoses posing a potential tripping hazard.
Licensing Specialist observed three Emergency forms not complete during the inspection.
Upon arrival of inspection, LS observed 9 children in care. Four children were school age children which were the Provider's grandchildren. Corrected during inspection. One child under the age of two was picked up at 12:51pm.
Licensing Specialist did not observe a gate secured at the bottom of the first floor steps. Corrected during inspection. Provider secured the gate at the bottom of the first floor steps during the inspection.
Licensing Specialist observed two grills uncovered during the inspection. Corrected during inspection. Provider placed the covers over the grills during the inspection.
Upon arrival of inspection, three children were sleeping on a pillow on the floor. Corrected during inspection. Provider placed the two children on their sleeping bags and one child was picked up by their mom.
At 12:52pm the Provider went to the second floor of the home to check on her grandchild using the bathroom. Corrected during the inspection. The Provider returned to the first floor where the children were at 12:56pm.
Licensing Specialist observed the Provider leave the dining room where three children, one under the age of two, were sleeping in order to check on her four grandchildren in the living room. The Provider is not able to see the children in the dining room sleeping while in the living room unless she stands directly in the hallway between the two rooms. Corrected during inspection. The children all woke up for lunch and the Provider was able to hear all children.
