At the time of the visit all medication outlined in IHCP's were not at the program. Two children's plans included Benadryl and parents did not provide Benadryl.
At the time of the visit the blinds in the older toddler classroom were in disrepair and were down and a hazard to the children. program was cited previously for this.
At the time of the visit there were some water cups used by the children throughout the day that were not labeled with the children's names.

Floor in one of the toddler rooms has a brown stick-on carpet over the floor. The floor has visible dirt on it under the brown carpet. Flooring in the infant room and one of the toddler rooms is chipping and in need of repair. There are blinds in one of the toddler rooms that need to be taken down due to them being in disrepair. One toddler room has a metal threshold between the carpet and the flooring. The metal was sharp in some areas and is a hazard. Gate between the infant and toddler room latch does not work on both sides.
Broken little tykes basketball hoop was observed. Pink and grey plastic play pen was taken apart and had some small pieces accessible to children on the playground.

Paint throughout the program had some areas of chipping. Shades in the older toddler room were observed with visible rips on them and stained.
Infants have a small, enclosed area that does not meet the 75 square feet per child. Infants do not have sufficient equipment accessible for use on the playground.

At the time of the visit there was a climber on the playground that did not have the adequate depth of impact absorbing material. The climber did not have the proper 6 feet around all sides a child could fall from. At the time of the visit there were infant swings being used that were not anchored down and did not have the proper fall zones or depth of impact absorbing material.
At the time of the visit the door molding outside accessible to the children was rotting and in disrepair.

At the time of the visit the following hazards were observed on the playground. Pieces of a picnic table under the infant swings. Broken sand toys throughout the playground. Broken water table A shade canapy in a bag on the ground was a tripping hazard. Tarp over the infant swings was ripped and in disrepair. Siding on the building on the playground is coming apart in some areas and sharp. The shed had a sharp slice in the front accessible to the children. A shovel, part of a umbrella and a tarp were observed on top of the shed. Rake, shovel and broom accessible to the children on the playground. A cracked cover to a storage bin accessible to the children. Gates and a broken table stored behind strollers accessible to the children. Plastic bags on the ground in the playground. Strollers hanging on the fence and children could hit their heads. Two hoses were observed on the playground and were a tripping hazard. A metal black and decker work bench. Little Tykes orange and cream picnic table was in disrepair.
At the time of the visit the following was observed: Peeling paint throughout the indoor space. Bathroom in the preschool classroom had a vanity that was in disrepair and appeared to have water damage. The molding behind the toilet in the preschool bathroom appeared to have water damage. Air conditioners in the infant and preschool rooms had dust on them. A broken infant swing was observed in the infant classroom. The faucet in on bathroom had rust and needed to be replaced. Metal paper towel holders in some places are rusting and paint is chipping. Mops in the bathrooms accessible to the children. Changing mat in one of the bathrooms on the floor. Please store on a hook or a shelf. Changing mat in the infant classroom had a soft cover over it which cannot be used in Large Group Child Care.

Building and fire inspections were expired at the time of the visit.
At the time of the visit there was a wooden washing machine from home living that was tippable and not secure along with other items in the classrooms. Please Ensure that all equipment not on wheels is secure. Cords In the infant and preschool classrooms were hanging accessible to children. Felt board in the preschool classroom was in disrepair. Urine odor in the bathroom off the infant classroom.

At the time of the visit the diaper changing mat in the older toddler classroom is in the bathroom and was observed on the floor. Please ensure that the diapering mat is stored off the floor in the bathroom.
At the time of the visit not all IHCP did not include who trained the staff in the child's medical condition and who is trained at the program.
At the time of the visit not all of the required parental and doctor signatures were complete on the Individual Health Care Plan.
At the time of the visit there was an area in the older toddler room that had an old carpet area that was covered over with a plastic floor mat. The floor around the area had visible dirt as well as the carpet under the plastic covering. There is an area of the tile in the older toddler room that has stuck on sticky residue from old duct tape. This area is in need of professional cleaning.
The exterior of the building had areas that had peeling paint. At the time of the visit the following was observed. Chipping pain on the exterior of the building, a rotted post to secure the gate to the playground.
