At the time of the visit the program did not have (2) verbal written references in the educators files. The program emailed the references however they did not speak to the individual.
At the time of the visit two educators were not supervising the children during rest time as they were on their cell phones.
At the time of the visit the program had an expired bottle of Benadryl for a child with an IHCP, and the program did not have a doctor's note for a couple over the counter medication.

At the time of the visit the emergency exit doorframe in {REDACTED} was broken with metal loose and concrete broken. The broken pieces are accessible to the children.
