At the time of the visit the toddler sleep mats with the sheets were on the floor and the educator stepped on the sheets with their shoes not keeping the sheet in a sanitary manner.
At the time of the visit the following was accessible to the children: knife in a cabinet draw in the {REDACTED} classroom, a wire longer than 6 inches was near a child sleeping in a crib, the chairs in the toddler classroom were stacked, and an educator purse was in an unlocked cabinet in the Kindergarten classroom.
At the time of the visit the program did not have a behavior intervention plan or documentation of a behavior incident for a child.
