At the time of the visit the program had several children files that were not current.
At the time of the visit one educator did not have a completed BRC.
At the time of the visit the following hazards were accessible and within reach of the children: wire to the lamp longer than 6 inches, a shelf was not bolted to the wall in the nap area, and Clorox wipes on a shelf within reach of the children.
At the time of the visit the program did not have the original box with the prescription label on an EPI pen and (1) bottle of Benadryl was expired.

At the time of the visit the program did not have (1) BRC completed and (1) educator did not have a BRC on file.
At the time of the visit the following was expired: (1) antihistamine, inhaler, and EPI pen was expired.
At the time of the visit the educator did not wash their hands and did not change their gloves during the process of assisting a child changing their soiled clothes and cleaning their dirty shoes.

At the time of the visit one educator stated there were 19 children however on the attendance there was one child not signed in and only 18 children present.
At the time of the visit the building inspection is expired.
The program did not have all the required information on the practice fire drill log. The program needs to ensure the date, time, number of children, route, and effectiveness are on the log.
Two of the IHCP did not have the parent signature and (1) EPI for a child with a life-saving medication did not have the medication in the classroom.
At the time of the visit there were (3) educator purses avialable and accessible to the children.

In two of the classrooms the attendance did not accurately reflect the number of children present as several children were not signed in to the classroom.
The schedule was not posted in an area visible to parents and visitors.
Several of the programs did not have the evacuation procedures posted at each exit.
The program is not supervising the children when the use the bathroom facilities allowing more than one child to use the bathroom at a time.
