Indoor Hazards
Licensor discussed with Educator the need for a list to be posted of all emergency or lifesaving medications, including but not limited to epinephrine auto-injectors, inhalers, and anti-seizure medications that specifies to which child they belong. Licensor also discussed that this information needs to have a cover sheet in order to protect the child’s confidentiality. There is a child in the program who has [REDACTED] and is prescribed [REDACTED]. Discussed that this information needs to be posted confidentially.

All of the required items were observed in the first aid kit, except for tweezers.
Educator has an up to date evacuation log. However, for the April fire drill that was conducted on 4/24/25, the majority of the required information was not filled in.
Licensor inspected all of the licensed, indoor space during the renewal visit. Educator stated the children rarely ever go to the 1st floor. Because of this, the decision was made to remove the 1st floor livingroom and kitchen from the licensed space. Moving forward, the only room on the 1st floor that will be licensed is the livingroom. The only hazards observed during the visit were the 2 air conditioners on the floor in the livingroom.

The bathroom is equipped with liquid soap and paper towels for hand washing. Educator stated the children's drink bottles are washed in the same sink in the bathroom, where the children wash their hands.

There are multiple exits from the home. All exits were clearly marked and most were easily accessible. However, the bulkhead exit in the basement space was not accessible. There was a chair in front of the door and 2 rolled up rugs on the stairs that lead to the bulkhead.
The only hazard that was observed in the fenced in back yard was the fire pit, which needs to have a cover.
Progress Reports

Licensor reviewed 12 of the children’s progress reports. Of these, 10 were complete and up to date. However, the other 2 reports were both out of date. One of these was a school age child whose annual report was due in February and the other was an infant whose report needs to be completed every 3 months.
Licensor inspected all of the indoor, licensed space during the monitoring visit, as well as the outdoor playspace. The following hazards were observed: **LIVING ROOM** - dresser that is a tipping hazard **DINING ROOM** - no hazards **KITCHEN** - one drawer has multiple hazards inside and the child safety tab isn’t working **PLAYROOM** - Educator would like this room removed from the licensed space as it is no longer used. ** BATHROOM **- no hazards **BASEMENT: FAMILY ROOM** - no hazards **ACTIVITY AREA** - no hazards **BATHROOM** - no hazards **OUTSIDE** - no hazards.

The following hazards were observed: ** KITCHEN - Lysol wipes on the counter ** LIVINGROOM - no hazards ** NEW PLAYROOM - no hazards ** MASTER BEDROOM - no hazards ** OUTSIDE - Plastic play set that does not have a fall zone. However, Educator stated she is getting rid of this piece of equipment. A gate needs to be added at the bottom of the stairs to the deck and the top.

Licensor inspected all of the licensed, indoor space during the renewal visit, except the old playroom. Educator asked that this room be removed form her license as she does not use it any longer. The following hazards were observed in the licensed room: In the kitchen, there was a knife block on the counter. However, Educator moved this to above the fridge during the visit. There was a dress up closet in the new playroom which is a tipping hazard and needs to be anchored. Outside, Educator has a fenced in backyard, which is used as the children's playspace. There are 3 separate plastic slides which are not included in the fallzone area, but do require a fallzone as they are over 18 inches high.

Licensor reviewed 6 of the children's records. Of these, 5 were complete and up to date. The other record had an out of date physical.
Educator utilizes the fenced in backyard as the outdoor playspace. There are several small slides and swing structures that Educator uses. Several structures have a fallzone in a mulched area. However, there is one structure that has 2 baby swings and a slide. This structure does not have a fallzone.
