Indoor Hazards
Licensor inspected the licensed space on the 1st floor of the home. The following hazards were observed: ** PLAYROOM **- floor lamp is a tipping hazard ** BEDROOM **- 2 floor lamps are tipping hazards. All lamps need to either be removed or secured to the wall.

Educator did not conduct a fire drill in May. Educator also needs to alternate the exit used during the fire drills more often. So far this year, Educator has used the same exit for every drill, except for 1.
Licensor randomly reviewed 4 of the children's records. Of these, 2 were complete and up to date, 1 had an out of date physical (school age child but does not have the school age section filled out in the enrollment packet), and the 4th record had out of date signatures in the enrollment packet. Space (indoor / outdoor hazards) - R Apr 2025
Licensor inspected all of the licensed, indoor space during the monitoring visit, as well as the outdoor playspace. The only hazards that were observed were outlets that were not covered in the 1st floor bedroom and the basement kitchen.

Licensor reviewed 5 of the children's records. Of these, 3 were complete and up to date. The other 2 records both had out of date physicals.
Educator conducted a fire drill every month except for last month (August). In the last 8 months, Educator only exited through the first floor once so far this year. Please ensure that the exit used for the fire drills is alternated each month.
Of the 5 childrne’s records that were reviewed, 3 did not have written notification of firearms in the home.
Educator could not locate thermometer in the fridge during the visit. Please ensure that the temperature is between 32F-40F degrees.
No immediate health risks were observed during the renewal visit Tested the smoke detector in the basement and it worked appropriately. There are no carbon monoxide detectors.

There are multiple exits from the home, including the rear slider in the basement, and the front door. Educator did not have an exit sign on the rear slider.
This investigation inspection recorded no violations or advisories.
Educator does not transport the children. However, there is 1 child who is transported by bus to the program and Educator has to go outside to get the child off of the bus. Educator needs to ensure all of the families have signed the Indirect Supervision form, so that the other children can be left inside the program space, while she goes outside to get the child off the bus.
