Educator must maintain current certification in CPR & First Aid
The attendance record showed 16 children present, while there were 12 in the classroom. When this was addressed with the staff, they were unsure of which of the 16 children were actually present in the classroom. The program was cited on this in a prior visit with guidance on how to correct the attendance records.
The staff has not maintained current first aid certification. Currently, there are only two staff members with current first aid certificates. This was cited during the previous visit.
While reviewing IHCP's it was noted a child was attending with a [REDACTED] but did not have an IHCP on file.
A review of a staff members file, showed her BRC to be expired.

Upon review of attendance it was found to be inaccurate. Additionally, each classroom was not maintaining a list of children in their care.
Upon review of the evacuation drill log, it was noted that it did not contain all required information.
Upon review of medication storage, an [REDACTED] was observed to be stored on a counter top within reach of children.

False and Misleading Informaiton
Child guidance practices by an Educator in the younger grouping of children were completed in a manner that did not maximize the growth and development of the children. The Educator used a deficient based approach, directed children with comments, such as, "you need to stop", "do this now", and "stop". This Educator did not provide any directions of what was expected behaviors or options that were available to the children to engage in.
During the time of the visit, the Licensing Specialist observed the program not meeting staff/child ratios at multiple times throughout the visit. This occurred when: 1) An Educator from the younger aged grouping left the classroom to meet a child at the front door who was arriving off a van, leaving the 2nd Educator in this classroom to supervise more than 13 children. 2) The program had to evacuate during the visit, due to a "carbon monoxide" alarm, and the Program Administrator did not go outdoors with the groupings of children as he needed to investigate why the alarm was occurring 3) When an Educator went to the front door of the building to pick up a grouping of 17 children alone who were being dropped off by the public school bus The program was also cited for not being in compliance with child/staff ratios during a September 2019 monitoring visit.
The Licensee did not provide a well balanced, curriculum of specifically planned activities for the grouping of children who are in kindergarten, first and 2nd grade. The curriculum was not planned to engage and encourage the developmental needs and interests of children as the Licensing Specialist observed an Educator reading, "The Three Little Bears" to this grouping of children which was not linguistically appropriate for all of the ages/developmental needs of the children in this classroom, albeit the Educator required all of the children in attendance to sit on the rug and listen to her reading this story.
Two of the four staff files that were reviewed at today's visit were also reviewed during the September 2019 visit and seen as lacking evidence of compliance at that visit as well as during today's visit. One of the staff files reviewed today was of a newly hired staff. The Program Director provided false and misleading information as he indicated as a response to the September 2019 staff record non-compliance corrective action plan that all staff records were updated with the necessary paperwork and the Program Director would ensure that all newly hired staff would have the necessary evidence of documentation prior to being hired.
Through a review of four staff files, the following was noted: 1) Three files did not have evidence of an application or resume 2) Three files did not have evidence that verbal references were completed 3) Three files did not have evidence of the interview dates 4) One file did not have evidence of a current PQ registry 5) One file did not have evidence of [REDACTED] . 6) One file did not have evidence that the staff was orientated to the program albeit this staff was working with children in the program on the day of the visit. 7) One staff did not have any evidence that bi-monthly observations or an annual evaluation were conducted consistently at the required frequency needed.

The attendance record for the younger children grouping classroom was not accurate as there were 19 children present and only 17 children signed into the classroom.
The Program is not being soundly administered as on the day of the visit the following specific program responsibilities were not being upheld: 1) A newly hired staff person was not aware of the emergency evacuation procedures when a carbon monoxide detector was sounding 2) The Child/Staff Ratio is not being constantly maintained 3) The program's child guidance and curriculum policies are not being followed in practice, by all of the staff
At the time of the visit, Educators in the young aged children's classroom were not quick to intervene when two boys were observed chasing each other around the classroom in an aggressive manner and were able to wrestle with each other on the floor for a few moments before their behavior was addressed.
Through a random review of three children's individual health care plans, it was noted that the IHCP's did not have all of the required information. The program was cited for this same non-compliance during a visit conducted at the program in September 2019 visit.
At the time of the visit, children's coats and backpacks were seen strewn on the floor and a table within the classroom that the program is not using, and where there are coat hooks for the children to hang their belongings. The Program Administrator and Educators were seen supervising children while they placed their belongings in this manner and did not model healthy behaviors by asking the children to hang up their belongings on the hooks provided instead of throwing them on a table, which some of them inadvertently fell off from and landed on the floor.
