Medication Administration Policy
Licensor observed that the pellet stove was not properly padded (the front of the stove was uncovered) and although she had a gate but it was not secured and could be pushed on and the gate would collapse to the ground.
Licensor observed that the program's medication administration policy was incomplete.
Educator’s expiration date for: CPR: 11/30/19 (expired) FA: Licensor did not observe evidence of the first aid certification. Note: The educator stated that she registered to take a class in February 14, 2020 for the renewal of the CPR/AED and First Aid. She also expressed that the {REDACTED}, does not has have CPR and First Aid certifications. Discussed that the educator would submit the registration information to the Licensor of the class.
Heating Safety
Licensor observed, the following health risks accessible to the children: 1) a crystal bottle of perfume in the educator’s bedroom. 2) hair products in the educator’s daughter's bedroom.
Space (indoor / outdoor hazards) - R Apr 2025
Upon arrival, Licensor observed the following: 1) The pellet stove was still on. The educator stated that she turned it on this morning to heat the lower level playroom for the children. Although the educator stated that it was turned off, Licensor observed the pellets were still burning. 2) Licensor touched the side of the pellet stove and it was warm.The stove was not sufficiently cooled off to prevent a child from getting burned. 3) Licensor observed that the pellet stove, was not barricaded at least three feet in height and installed at least three feet from the stove. Note: On 6/19/14 the educator's current Licensor, Maristela Tosato, conducted a Renewal visit. The report from this visit stated the following: "Non-compliance (NC): educator needs a permit for her pellet stove on lower level. She needs to pad bricks and pellet stove when daycare children are present. She stated she is not going to use the pellet stove during daycare hours". The educator Action Plan stated the following in writing: "I have since paded bricks with pillows of pellet stove with blanket". Note: Licensor Eliliana Javier observed, that a permit has not been received by EEC for the pellet stove after revising the educator's paper file.

After reviewing four children’s records, Licensor observed the following: 1) one child’S progress report, was missing the year that it was completed.
After the review of four children's random records, Licensor observed the following: 1) one child was missing days and times in care in the enrollment packet. 2) two of the children were missing the physical examination. 3) one child was missing the {REDACTED} result.
Upon arrival, Licensor observed, a dog that was sleeping in his large bed behind the door of the exit located through the kitchen.The exit was not accessible in case of an emergency due to being blocked by the dog and his bed. Note: The two other exits (one in the front and another through the lower level) were accessible and safe.
Licensor observed, the following indoor hazards accessible to the children: 1) unlocked cabinet under the bathroom sink with cleaning products (wipes and Clorox spray). 2) unlocked drawer under the bathroom sink with three nail clippers. 3) unlocked drawer under the bathroom sink with hair products. 4) unlocked standing cabinet shelf in the bathroom, with various hair products. 5) loose cables in the three bedrooms, some of which were long extension cords and plug in phone chargers, that represent a strangulation and tripping hazard for the children.
