The program had an educator start employment prior to completing the BRC process with fingerprints.
In the infant/Toddler classroom the bathroom is cluttered where the sink is not easily accessible for the children to wash their hands and the classroom is cluttered with equipment. In the Preschool classroom there was a pack and play half folded up as the children were playing around it.
The following documentation was expired and/or missing in the staff record: (2) expired PQ registry, (1) PQ registry missing, (3) Physicals expired, and Bi-monthly observations are not consistently being conducted and documented.
At the time of the visit, the program was not able to provide a current and up to date Staff Record Checklist.
An educator assisted a child in the Preschool classroom with wiping the child's tears and nose however the educator and child did not wash their hands.
At the time of the visit one child's [MEDICINE] was expired.
In the classroom there were plastic bags and a container of Clorox wipes accessible and within reach of the children.
