Licensor observed a plugged white phone charger under the kitchen island in the first floor level accessible to children, which presented a strangulation hazard.

Licensor observed the following was missing from the Emergency Preparedness plan: 1) “Lock down” procedures
Upon Licensor’s arrival, the educator was found alone caring for seven children under the age of {REDACTED}, including four children under {REDACTED} (one{REDACTED} and three {REDACTED}).The educator did not make sure the program was staffed by the appropriate number of persons with experience and/or education in providing care to children.
After the review of three regular assistants records, Licensor observed the following: 1) two assistants were missing up to date PQR. 2) an assistant was missing evidence of PQR. 3) an assistant was missing physical examination as required. 3) an assistant was missing evidence of ongoing professional development as required. 4) an assistant was missing up to date ongoing professional development as required. 5) two assistants were missing evidence of the EEC Essentials required to be completed by September 2019.
Licensor observed, the educator did not have a pressure gate at the bottom of the indoor stairs in the first floor leading to the upper level unlicensed space.

After the review of five children’s random records, Licensor observed the following: 1) five children were missing the current licensed assistants as authorized to take the children on off site activities. 2) five children were missing a current list of assistant authorized to administer first and CPR and take the child to the hospital. 3) two children were missing up to date {REDACTED} results in the file. 4) a child was missing the admission date in the enrollment packet.

Licensor observed regular assistant {REDACTED} had arrival time for today 3/11/20 , but not departure time when he was allowed to leave the program to buy toilet paper. In addition, the review of the attendance from the previous week did not show arrival and departure times for any of the assistants.
Licensor observed the educator did not document the effectiveness of the drill, including how many minutes it took, for the last fire drill on 2/10/20.
Licensor observed the following: 1) four children were missing the Family Contact information/Emergency cards in the first aid kit. 2) four children had expired Family contact information/Emergency cards in the first aid kit.
The educator did Not take into consideration the number of educator’s when she was found alone caring for seven children under {REDACTED}, including four children under {REDACTED} (one {REDACTED} and three {REDACTED}).
Licensor did not observe evidence of progress reports for any of the children currently enrolled in the program.

The educator did not use her good judgement when she allowed her assistant to go out to buy toilet paper, and she stayed alone caring for seven children under {REDACTED}, including four children under {REDACTED} (one{REDACTED} and three {REDACTED}).
Licensor observed the following indoor hazards accessible to the children: 1) seven uncovered outlets in the first floor level (kitchen and living room). 2)an uncovered trash can inside the bathroom in the lower level/basement. Licensor observed the following outdoor hazards accessible to the children: 1) an uncovered propane gas tank on the ground. 2) three shelves, including two metal, accessible to the children , which presented a tipping hazard. 3) outdoor equipment ( leave blower vacuums, and a pressure washer machine).
