At the time of the visit, accurate attendance was not maintained as departure time was not documented for a child no longer present in the group. The daily attendance reflected three children; however, two children were present in the group.
An individual health care plan for an emergency prescription medication (Epi-Pen) was not available for review for a child diagnosed with a chronic medical condition.

1. At the time of the visit, a current staff schedule was not posted on site and was not available for review. 2. EEC found insufficient evidence to support teacher qualifications as documentation was not available for review.
An allergy list posted on site was found to be outdated as it did not indicate medical information of three additional children diagnosed with emergency/lifesaving medications and or allergies.

This provisional license renewal visit inspection recorded no violations or advisories.
The program's daily written attendance records did not indicate each child's arrival and departure times. Attendance is also taken through the use of an IPAD which parents are to use at pickup times.

1. At the time of the visit, a staff’s record file was not available review for a staff member currently working at the program for approximately two weeks. 2. Documentation of an annual staff performance appraisal/evaluation was not available for review for two staff members hired on 3/2023 and 11/2023.
A review of staff's background record check reflected a staff member working in the before school program with an expired fingerprint status as of 9/19/2022.
Appropriate group size was not maintained as thirty-five children were combined in a group inside the cafeteria during story time. This included a {REDACTED} mixed age group and a {REDACTED} fixed age group.
An updated fire inspection was not available for review, last inspection on file dating 9/27/2023 was found to be outdated as of 9/27/2024.
Toxic Substances
Documentation of a current first aid certification was not available for review in a staff's record file, last record on file reflected expiration date of 2/18/2023.
Emergency/evacuation written procedures/diagram was not posted at an exit within the cafeteria space and at a second exit within the gymnasium space.
A disinfectant spray can, and industrial trash bags were stored in an open cardboard box inside the boy's restroom, within reach of children.

A staff's First Aid Certification reflected expiration date of 7/10/2021.
A BRC review conducted via the BRC Navigator Portal reflected no BRC suitability status for five staff.
1. At the time of the visit, the group size ratio reflected 38 children present and gathered in the library for story time which exceeded the maximum group size. 2. The program is licensed for 10 {REDACTED}mixed group, however the {REDACTED} group reflected 13 {REDACTED} children.
At the time of the visit, the program was observed to be operating and providing care for more than the maximum number of children indicated on the license certificate by two children.
The emergency #'s posted on site did not include documentation of the telephone number from the Poison Control Center, name and telephone number of the emergency back-up person, program name, and program address.
A review of the FY 2022 emergency evacuation drills did not contain documentation of practice drills conducted between the months of January through June of 2022.
The allergy list posted on site indicated a child diagnosed with {REDACTED} and requiring an {REDACTED} medication, however an {REDACTED} medication was not available for review at the time of the visit.

The garbage bins used by the program did not contain covers. The {REDACTED} reported that the garbage is emptied daily.
The First Aid kit was incomplete as it did not include a thermometer.
The temperature from a thermometer reading in a refrigerator used to store food readings were at 28 degrees at the time of the visit.
In the cafeteria, peeling/flaking paint was observed on the wall below the window area. The {REDACTED} reported that maintenance personnel will be repainting the facility during the summer vacation.
