The licensor reviewed a sample packet of children records and observed the following: 1)Child #1 In enrollment packet, missing information on the general permissions and medical consent forms needs permission for consent to be provided for assistants. And has an expired physical report, immunizations last. And lead test is needed. Child #2: In enrollment packet, missing information on the general permissions and medical consent forms needs permission for consent to be provided for assistants. And Physical examination and immunizations have been expired since 1/7/2024. And lead test is needed, last one on file is from 7/2/2024. Child #3: In enrollment packet, missing information on the general permissions and medical consent forms needs permission for consent to be provided for assistants. And lead test on file is expired, since 6/25/2024. Child #4:In enrollment packet, missing information on the general permissions and medical consent forms needs permission for consent to be provided for assistants.
