This investigation inspection recorded no violations or advisories.
Licensor reviewed the training certificates presented by the educator for the current licensing cycle 2016-2019. The training hours are distributed as follows: 2019 - 14 training hours (EEC Essentials) 1. The educator needs 16 training hours as required for license renewal. 2. Licensor was not able to verify 37 hours of training (ADHD 10 hrs, Children with Special Needs: Autism Spectrum Disorder Student 16 hrs and Appelbaum Training Institute 11 hrs), presented by the educator at the time of the visit, because the original certificates for the trainings were not available on site.
Licensor reviewed 10 children’s records. The following information is missing: 1. Transportation Plan section on all the enrollment packets. 2. Child {REDACTED}: Parental Permission for medication administration {REDACTED}.
PQR #9500219 Exp. 4/22/20 PQR only shows 24 hours of training. Only 14 hours of training were confirmed by this Licensor.
