A review of four staff files, Licensor noted the following: 1. One staff had an expired PQR dated 7/11/24. 2. Two staff were missing evidence of physical examinations and MMR immunizations. 3. One staff had a blank staff form and was missing evidence of program orientation.

A review of two IHCP plans found that it was incomplete: The both plans were missing the potential side effects of treatment, potential consequences if treatment is not given and who has trained the staff and will be administering the treatment while the child is at the program.

The Evacuation drill log was missing the month of April.
At the time of the visit a review of medications found that 1. A child’s asthma action plan was signed on 7/26/2021. Asthma action plans need to be updated annually. 2. The Individual health Care Plan (IHCP) plan was not signed or dated by the Health Care Practitioner. 3. A child had a hydrocortisone cream but did not have consent form for authorization.

At the time of the visit there were 26 children on the attendance however, when EEC arrived at the program one child was being dismissed. EEC told the site coordinator that only children that is enrolled in the program should be attending.
This provisional license renewal visit inspection recorded no violations or advisories.
Staff schedule was not current and was not posted.
1. One prescribed medication (DEA) was stored in the first aid bag and not in a locked cabinet. 2. One child had an expired medication dated 7/2023.
