Second emergency exit from the basement is missing a working light and had a bucket on the path.
At the time of the visit the infant was awake. Educator was using a crib sheet for the port-a-crib.
Educator has been washing hands after diapering in the kitchen sink where food is being prepared.

One child was observed sitting on a chair that the backing was broken. TV in the living room is not secured, two glass panels on the living room french doors are broken. Fence in the backyard is down, there was litter in the backyard (paper, empty water bottles)
Toothbrushes in the bathroom were stored inside of toothbrushes storage cases. Lanyard used for the form of identification does not come apart by pulling - chocking hazard.
