At the time of the visit the 3-year-old classroom evacuation log showed only 1 drill for the entire year (March 2026). It could not be determined if evacuation drills were being practiced every month with all groups of children.
Attendance was not accurate in either classroom; 8 children were present in the 3-year-old room and no children were signed in on the attendance sheet, and 18 children were present in the 4-year-old room and only 17 children were signed in on the attendnace sheet.

When reviewing IHCP's the Licensor noted they were missing required information; The name of the Educators who were trained in the child's IHCP and the name of the person who trained the Educators.

The Teacher qualified Educator in the 3's classroom was observed leaving the room two times to bring children to the bathroom, leaving the Asst. Teacher who was a substitute alone with the group of children. Asst. Teachers must be under the direct visual supervision of a qualified Teacher at all times.
2 staff files were randomly reviewed, and the following was noted: 1. 1 file was missing an application/resume, verification of 2 verbal references, evidence of [REDACTED], and documentation of a program orientation. 2. 1 file was missing evidence of [REDACTED] and documentation of a program orientation.
When observing in the 3's classroom the Licensor noted the following: 1. A child blew his nose and did not wash his hands. 2. The table was only washed with a diaper wipe prior to the children eating snack; it must be washed then sanitized. it is always a 2-step process.
When observing in the 4's classrooms the Licensor noted containers of sanitizer/disinfectant and Clorox wipes and bottles of hand sanitizer stored on top of shelving units accessible to children.
At the time of the visit one child's IHCP stated to administer REDACTED] and there was none on site.
