IHCP's were reviewed and the following was noted: 1. 1 Educator stated a child had a sesame allergy but stated she was unsure if they had an epi-pen, the epi-pen was located in the classroom backpack. All Educators must know which children within the program have allergies and emergency medication and where the medication is kept. 2. 1 child's IHCP was not signed/dated by the parent.

When reviewing BRC's in the system it was determined 2 Educators were hired and working in the program without a current BRC on file, neither Educator was deemed Provisional or Suitable to work with children.
An Educator placed an infant on a boppy pillow, sat next to the child on the floor and fed the child a bottle.
1. An Educator in Infant 2 did not wash the diaper changing surface appropriately, she only used disinfectant, she did not wash the surface first with soap/water as required. 2. An Educator in the Pre-K classroom was using the IPAD with gloves on her hands, she then served Cheerios with her hands without washing them.
Diaper cream was found stored on a shelf accessible to children in the bathroom.
This investigation inspection recorded no violations or advisories.

Written Emergency Evacuation Procedures
When observing in classrooms the following was noted: 1. A plug adapter in the Toddler 2 room did not have safety protector outlet covers. 2. Cords to an IPAD and speaker was accessible to children in the Toddler 2 room and a speaker cord in PS 1. 3. Dramatic Play furniture was not secured in the Infant/Toddler classroom.

3 children's files were reviewed and the following was noted: 1. 1 child was missing evidence of an updated lead screening. 2. The program is switching over to electronic records, it could not be determined when 1 child's First Aid emergency consent form and Off site consent form was signed/dated due to the program not knowing how to print it out/access it on the computer.
3 staff files were reviewed, and 2 files were missing evidence of yearly evaluations.
Written emergency evacuation procedures and the diagram of escape routes were not posted by the back door that leads to the auditorium in the Infant/Toddler classroom.

When reviewing medications the following was noted: 1.1 child's IHCP stated to administer [REDACTED] and there was none on site. 2. 1 child's bottle of [REDACTED]had expired 1/2024. 3. 1 child's prescription label stated to discard after 10/6/2023.
When observing in the Infant classroom the Licensor noted paintbrushes were in the hand washing after diapering sink.
The doors to the diaper changing table cabinet in the Toddler 3 classroom were not locked, diaper creams were stored in the cabinet accessible to children.

At the time of the visit the following was noted in the Toddler 3 classroom: 1. They did not have an appropriate amount of toys/materials out on shelves for the children to play with, several shelves were empty. 2. There was no dramatic play table and chairs in the housekeeping area to foster dramatic play.
When observing int he toddler 3 classroom the Licensor noted toy containers with covers, this is not considered available and accessible to the children.

3 staff records were reviewed, and the following was noted: 1. Verification of 2 verbal references was missing from 1 file. 2. 1 file was missing evidence of an updated PQ registry and evidence of the required EEC 12 Essentials trainings.
At the time of the visit snack items cereal, pretzels, Tostito chips etc...) were open and rolled closed, they were not in a sealed container or ziplock bag.
Educators in the Toddler 3 classroom did not follow the program's snack menu which stated pancakes and fresh fruit, they served pancakes and water for morning snack. This is not considered meeting the USDA snack guidelines.
When observing in the Infant room the Licensor noted the Educators did not wash their hands or the children's hands prior to feeding a bottle.
A piece of wooden dramatic play furniture in the Toddler 3 classroom was not secured and potentially tippable.
When observing diapering in the Toddler 3 classroom the following was noted: 1. The Educator took both hands off of the child who was being changed on an elevated surface. 2. The Educator did not wash her hands after diapering a child and cleaning the surface.
When observing in the Toddler 3 classroom the following was noted: 1. The Educator did not allow the bleach/water solution to remain on the table long enough to sanitize, she wiped it dry. 2. The Educator did not allow the bleach/water solution to remain on the diaper changing table long enough to disinfect, she wiped it dry so that she could change another child immediately.

At the Change of capacity visit the following was noted: 1. Some pieces of furniture were not secured and potentially tippable.

When reviewing medication and IHCP's the Licensor noted 1 child with [REDACTED] had a medication order on file for an [REDACTED] but did not have an IHCP as required.

Rest mats were stored stacked with the sheet/blankets still on them which resulted in the sheets/blankets touching one another which is not considered sanitary storage of bed linens.
At the time of the visit bags of snacks were found rolled closed and taped, they were not in a sealed container as required.

Of the 3 children's files randomly reviewed the following was noted: 1. 1 child's file's face sheet, 1st aid /emergency medical consent, transportation plan, and developmental history had expired 1/2023, there was no evidence of a [REDACTED]and the progress report was not dated so it could not be determined when it was completed. 2. 1 child's file was missing a face sheet and progress report.
When observing in classrooms the following was noted: 1. The PS dramatic play kitchen was not secured and potentially tippable. 2. Dyson vacuum cleaners were found leaning against the wall and not secured int he Toddler and T/PS classrooms.

At the time of the visit the metal surround around the gas fireplace that is located in the front entrance was hot to the touch. The Director immediately turned the fire place off. The fireplace must be kept off during childcare hours or made inaccessible to children.
