At the time of the visit attendance was not accurate for over 20 minutes when the upstairs group combined with the lobby group.
1 child with an[REDACTED] did not have an IHCP on file.
1 child's medication consent form was not completely filled out by the parent; it was missing required information.

Window guards
At the time of the Change of Space visit there was not window guards installed on windows in Rm 2,3 and 4 which are all on the second floor.

Staff files were reviewed, and the following was noted: 1. 2 files did not contain evidence of a program orientation. 2. 4 were missing the verification of 2 verbal references.
1 child's IHCP was not signed/dated by the parent.
Ratios were not being maintained while the children were arriving and eating lunch in the big hall. The Licensor observed 46 children with only 3 Educators present.
Emergency medications readily available
IHCP's were reviewed and the following was noted: 1. 2 children's IHCP's had expired 8/30/24 and 9/14/24. 2. 3 plans were not signed/dated by the parent. 3. 1 child was carrying an [REDACTED] on her that the program did not know about and had no IHCP for.

The 6 original staff files were re-reviewed, and the following was noted: 1. 3 files were missing a staff information form. 2. 2 files were missing an application/resume. 3. 4 files were missing the documentation of interview date. 4. 4 files were missing the verification of 2 verbal references. 5. 4 files were missing the date of hire. 6. 1 file was missing evidence of qualifications. 7. 1 file was missing the PQ registration. 8. 5 files were missing evidence of a physical exam. 9. 1 file was missing evidence of [REDACTED] 10. 1 file was missing evidence of EEC essentials prior to working with children. 11. All files were missing evidence of a program orientation.
3 children's files were reviewed and the following was noted: 1. The date of admission was missing from all files. 2. The 1st Aid Emergency Medical Consent & Release form was missing from all files.
Drinking water was available however several children asked and they did not have cups outside for them to use to get a drink of water.
A child went out to the outside play area with his lunch as he didn't finish eating and was standing/sitting in the parking lot and grassy area touching the ground and then eating/drinking, this is not considered serving food in a safe and sanitary manner.
Emergency medications were reviewed and following was noted; 1. 1 child's [REDACTED]was not in its original box or with the prescription label affixed to it. 2. 1 child's prescription for an [REDACTED] stated to discard after 8/20/24 and their epi-pen stated to discard after 8/17/24.
At the time of the visit the following was observed: 1. Children were eating and coloring at the same time, this is not considered proper infection control. 2. Tables were not washed and sanitized prior to the children eating lunch due to them being covered in fabric.
Emergency medications were reviewed and following was noted; 1. 1 child's IHCP stated to administer [REDACTED] and there was none on site. 2. 2 children's IHCP stated to administer [REDACTED]and there was none on site.
This investigation inspection recorded no violations or advisories.
