Licensor inspected all of the licensed, indoor space during the visit, as well as the outdoor playspace (fenced in back yard). Inside, the only hazard that was observed was in the kitchen. There were sharp knives in the sink. Outside, there was dog feces inside the fenced in area in the back yard, where the children play.

Educator is licensed to care for 10 children, which requires Educator to have an assistant. However, due to the fact that Educator does not have an assistant, her licensed capacity must be decreased. There are currently 4 children enrolled in the program.
Licensor reviewed 3 of the children's progress reports. Of these, 1 was complete and up to date, 1 was overdue, and 1 had no report.

Licensor inspected all of the licensed, indoor space during the monitoring visit, as well as the outdoor playspace. The only hazard that was observed was dog food bowls on the floor in the livingroom/diningroom area.
Educator has an up to date evacuation log. However, the length of time it took to complete the drill needs to be included in the log.

The primary exit is the front door. This exit was clearly marked and easily accessible. The 2nd exit from the home is in the basement, through the garage. The garage is filled with boxes, bins, various items including a fertilizer sprayer, toys, coolers, disinfectant wipes, etc. The exit is not clear due to these items being in piles throughout the garbage and there is a fridge that is blocking the walkway to get out of the house.

Licensor randomly reviewed 4 of the children's records/ Of these, 3 were complete and up to date, but the 4th record had no evidence of lead testing. Please review all of the children's records and ensure they are all complete and up to date.
Licensor reviewed 4 of the children's records. Of these, 2 progress reports were complete and up to date, but the other 2 reports were overdue.
Educator is licensed for 10 children, but does not have an assistant.
Licensor inspected all of the licensed, indoor space during the renewal visit, as well is the outdoor playspace. The following hazards were observed: **LIVING ROOM/DINING ROOM** - Christmas tree was not anchored, power strip behind bureau has an outlet not covered **KITCHEN** - sharp knives, second drawer down to the left of the stove, sharpies and small chokeable items in drawer on the far right **GAME ROOM** - Christmas tree was not anchored, and there was a treadmill in the room. **OFFICE ROOM** - cleaning sprays in the diapering area **PLAYROOM** - no hazards **BATHROOM** - no hazards **OUTSIDE** - no hazards, fenced in area in back yard.
All of the required items were observed in the first aid kit. However, the children’s emergency contact forms need to be updated.

Educator informed Licensor that the children's progress reports are not up to date. Please ensure that these reports are completed as soon as possible and provide a plan of how the reports will remain up to date in the future.
There are 2 exits from the home, the front door and the garage exit in the basement. In the garage there is a snowblower that needs to be covered. There is also a lot of cluttered that should be minimized.

Licensor was able to inspect all of the licensed, indoor space during the monitoring visit. The only room that had any hazards was the Livingroom/Diningroom. There is a wire rack in dining area, which is a tipping hazard. In the outdoor playspace, there is a gap between the fence and both of the gates (on either side), which is an entrapment hazard.
There are 2 exits from the home, the front door and the garage door. Both exits are clearly marked. The front door is easily accessible. The garage is very cluttered, with items piled high up to the ceiling almost. These items pose hazards to the children.
