During an interview with the program director, it was revealed that staff members take photos of children using their personal cell phones for the program's website and social media pages. The program cannot guarantee that these photos will remain confidential or that they won't be used inappropriately. inappropriately.

The following was observed: 1. There was a missing piece of baseboard exposing crumbling plaster in the Toddler/Preschool room. 2. There was broken plaster on the wall in the Preschool Room. 3. There were stained ceiling tiles and water-stained floor tiles below indicating there is a potential leak in the ceiling or roof.
The program did not conduct evacuation drills for January or February.
The first aid kit did not contain a CPR mouthguard.
There was no thermometer in the infant refrigerator.

An educator in the program had a current suitable BRC at a different program, however, did not have a suitable BRC through the current program.
Due to the lack of important children's documents, such as consent to administer first aid, the licensee being unaware of a potentially life-threatening health condition for a student, and no disinfectant/sanitizer in the program, it is determined that the program is not being soundly administered.
The program did not have a children's record checklist to review. Only six of the 9 children enrolled had files. Of the six files, 2 were missing developmental history and 1 only contained record of immunizations and a physical.
The licensee left a classroom out of ratio, leaving an educator alone with 6 toddler and preschool aged children.
A swing set, not professionally installed and without the required fall zone was observed on the playground.
Through a review of one child's medical records, it was noted that the child has a chronic medical condition that the program was unaware of and therefore not posted.
1.The program did not have an adequate amount of disinfectant/sanitizer to properly sanitize/disinfect surfaces. 2. There was no soap available at the handwashing sink next to the diaper changing table.
Through a review of a child's medical record, it was noted they have asthma and require a nebulizer or inhaler. Neither of these were on site.
1. Small cotton balls were observed in the sensory table that presented a choking hazard. 2. A pair of adult scissors was observed within reach of children.
A review of on student's medical record showed the child has [REDACTED]however the program was unaware of this.
Upon reviewing all staff files, the following was noted: 1. All staff files were missing proof of orientation 2. Four staff files were missing proof of references 3. Two staff files were missing proof of completion of first aid. 4. One staff files were missing was missing proof of EEC Essentials 2.0 completion. One staff file was missing the staff form.
The program has not yet secured the shelving and dramatic play equipment as they are still refining the room arrangements. The baseboard was loose in one area of the I/T room. The ceiling tiles were stained in the Preschool room. There was a missing piece of baseboard in the Toddler/PS room.
The infant/toddler classroom is missing art, a cozy area, and science/sensory materials
The Toddler/Preschool room does not yet have storage space for children’s belongings.
