Review of medication consents found an expired consent for one child's epi-pen and one child's consent was not signed and dated.

Review of IHCPs found the following: one child had 2 IHCPs that had contradictory information and one child's IHCP states a prescribed inhaler, but the program does not have an inhaler for this child.

Two educators were observed only using one step in the 2-step cleaning and disinfecting process on diaper changing tables.

Random review of 4 staff files found all 4 files missing a staff information form, no documentation of program orientation. One file did not have a current physical or evidence of [REDACTED]
An educator's personal drink and food were stored on the counter within reach of children.
