The licensor measured the depth of wood chips in four areas under and around the climbing structure. Two out of four areas did not have an adequate depth of 9 inches in the use zones under and around the climbing structure. The climbing structure contains hazards, including protruding screws under the climber, sharp edges on both sides at the top of the green slide, and chipped, flaking paint on the rock climbing wall.

Outdoor Space
The outdoor space contained hazards. Children were using climbing equipment and swings that were not inspected by EEC. There was an inadequate depth (2 inches or less) of impact absorbing material (wood chips) in the fall zones under all outdoor equipment. The ground was frozen and contained snow under one climber that was located in a shady area near the woods.
This renewal - monitoring visit inspection recorded no violations or advisories.
During a review of staff records, it was learned one educator was missing the staff orientation checklist and references, one educator was missing an EEC Essentials Training certificate, staff information form, and health records, and two educators were missing a current PQ Registry.

Indoor Space
It was observed that the indoor (barn) and outdoor space contains hazards; the spinning wheel on the top of the cabinet could fall and cause an injury to a child and standing water in the bin poses a health and safety issue for children.

During a review of children's records, four children were missing physicals and at least two children were missing transportation plans.
It was observed that an extension cords and a crockpot with the glass lid was accessible to children in the animal barn. The licensee must monitor the environment daily to identify and remove or repair any hazards that may cause injury to children.

During a review of staff records, it was found that some documentation was missing, including physicals, EEC Essentials Training certificate, First Aid and CPR certification, BRC consent forms and suitability letter, and one staff was missing the staff orientation checklist and an application or resume.
During a review of the children's records, it was found that some of the required documents were missing, including physicals, immunization records, lead screening, medical paperwork, and the transportation plan and authorization.
It was found that the program is not documenting emergency evacuation drills. 7.011(07)(h) states that, The educator must hold practice evacuation drills with all groups of children and all educators from each floor level of the approved space at least monthly. Drills must be held during different times of the program day, and must use alternative exits. The educator must document the date, time, exit route used, number of children evacuated and effectiveness of each drill.
During the visit, it was found that one child was missing the doctor's IHCP for a chronic medical condition.
