Children's Records
Licensor reviewed three random children's files. Licensor found two expired physicals and one expired annual update form. Please make a plan to ensure all enrollment paperwork and medicals are updated annually.
Provider has an enrolled child with emergency medication. Provider did not have a medication consent form on file and medication was expired. Please acquire updated medical forms annually and make a plan to ensure rescue medication on the property never expires.

Provider utilizes Ages and Stages Questionnaire along with EEC Developmental checklists for progress reports. Licensor randomly reviewed three children's files and found two children's progress reports were completed at incorrect ratios. Preschool age children require a progress report every 6 months and Provider was completing annually. Please adjust your frequency to the following: For infants and children with identified special needs the progress report must be prepared every three months. For toddlers and preschoolers, the progress report must be prepared every six months. For school age children, the progress report must be prepared at least annually, at the midpoint of the child’s program year.

First Aid Kit contained all required medical items; however, emergency contacts were outdated. Please acquire updated emergency contact forms or get a new signature at the bottom to confirm old numbers are still accurate and in use.
During licensing visit a child used a training toilet in the living room and then utilized the sink for food prep and general use to wash hands. Please move the training potty closer to or in the bathroom so that children have 1 sink for washing hands after going to the bathroom and another for food prep.
Licensor measured water temperature at 127F. Please reduce to below 120F.
Licensor did a walkthrough of all indoor licensed space and found the following hazards: *Living Room: Litter box was in licensed space and accessible to children on route to bathroom. Please move the litter box to unlicensed space or fence off litter box to prevent participants from having access.
