Two staff records were randomly selected for review and the following was learned: 1) Educator A did not have evidence of having provided the program with a [REDACTED]records as required 2) Educator A did not have evidence of bi-monthly staff observations or a staff evaluation as required. 3) Educator B did not have evidence of a [REDACTED]records, a program orientation, application and references as required.

A staff schedule was not posted where staff, visitors and parents could review it, as required by regulation.
1) It could not be determined if food was being stored in a safe and sanitary manner when the thermometer in the refrigerator wasn't working as required 2) the program did not have a written documented snack menu as required.
Educator's drinks and the cleaning chemicals were stored in the reach of children, creating a potential hazard.
The program did not have written documented evidence of fire drills as required by regulation.
During a review of program medication, the following was learned: 1) Child A had an allergy action that was not signed or date by their parent as required 2) Child A had [REDACTED] that expired 1/2026 3) Child A had[REDACTED]" in their med bag without a consent form or instructions for what it should be used for 4) Child B had an allergy action plan that called for "Cetirizine" but that medication was not on site as required 5) Child C had an inhaler that expired 1/2026.
Emergency medication was stored in the reach of children when educators brought the "emergency backpack" into the gym space and hung it over a chair, creating a potential hazard to children.
Not all snacks were stored in a sealed container as required by regulation.

Three Children's records were randomly reviewed, and the following was learned: 1) one Child did not have written documentation of a current transportation plan as required.
2 staff records were randomly selected for review and the following was learned: 1) two educators did not have written evidence of having provided the program with a current physical as required or[REDACTED] records as required 2) one staff did not have written evidence of having completed "strong start" prior to working in the program as required.
A health and safety review was conducted in the program and the following was learned: 1) stained ceiling tiles in the program's "main space" creating a potential hazard to children.
This enhanced monitoring visit inspection recorded no violations or advisories.
Three staff records were randomly selected for review and the following was learned: 1) one staff did not have evidence of an application or resume on file as required 2) 2 staff did not have evidence of a physical within one year as required by EEC 3) 1 staff did not have evidence of [REDACTED] on record as required.

This investigation inspection recorded no violations or advisories.
During the visit, the Licensor and the Area Manager walked into the gym space, where a GL and an AGL were with a grouping of children. Both staff were observed to have pulled folding chairs out and were sitting on the sidelines, not engaging with children and talking together.

During the visit, the Licensor and the Area Manager walked into the gym space and saw three adults sitting in folding chairs, watching the children and talking together. Through a conversation with the Area Manager, it was learned that the third adult did not work in the program and the Area Manager was unclear who they were. By regulation, the program cannot mix with the public and only those that have a background record check can be present with children.
It was observed during the visit that the Site Coordinator left the AGL alone in the classroom in order to assist a parent in picking up their child. A certified person is required to be in the grouping at all times by regulation.
