Inspection of indoor space found stained ceiling tiles, classrooms where the soap at children's sinks has significantly dripped down onto the step stool and 1 soap dispenser broken off its holder.

Observations of all staff at least once every other month are not being done by administration.
There were 2 [REDACTED] for one child. One of the [REDACTED] was not in it's original prescription box and it did not have a prescription label affixed to the inhaler.
Review of IHCPs found the following: 1 child's plan was expired, 1 child's plan was illegible, no plan for 1 child's chronic condition, and 2 plans that did not contain all required information.

Emergency medications were stored on a shelf in a classroom. When an educator was asked if they went outside with children a clear answer was not provided.

Review of 2 staff files found: 1 did not have a current physical upon hire, no evidence of strong start essentials training and no evidence of program orientation.
Bags of potato chips found in classrooms opened and not sealed properly.
The following was found during health and safety inspection: tippable equipment, rusted areas around exit doors, and green felted mats used under sensory tables are a tripping hazard.
One child's IHCP didn't state any medication for treatment, but program has an [REDACTED] Another child's IHCP stated no medication, but patient summary states [REDACTED] but no information when to give treatment. Program also has [REDACTED] on site for this child.
Attendance in one preschool classroom stated 18 children were in attendance when only 17 were present. One child had left during circle time and that child was not marked out.

Review of 5 children's files found 1 file with a first aid/emergency consent not signed and dated by parent/guardian.
One of 2 staff files reviewed did not contain evidence of a current physical and immunity to [REDACTED]
It was learned one staff who has been working at the program for longer than 6 months does not have current first aid.
It was learned a staff person being used as Teacher qualified did not have evidence of qualifications on file.
During the visit it was learned a bottle of [REDACTED]part of a child's emergency medical plan, was stored locked in a classroom cabinet. There is potential for the medication to not be available in an emergency if it is locked away and/or not going with the child when their group goes to the playground.
During health and safety inspection the following was found: staff had personal drinks within reach of children, hole in tile under a sensory table, paper towel dispenser not attached to wall, tippable furniture in a toddler room, vinyl baseboard missing from back door area in toddler room,
Playground fence poles continue to have chipping paint accessible to children.
Administration was unable to locate evacuation drill log therefore it could not be determined if drills are conducted.
