Random review of 6 children's files. Educator is in the process of having parents sign new annual update form. One child's last physical on file was from June 2024.
No current progress reports were available for review. Written progress reports must be completed every three months for infants and children with special needs, every six months for toddlers and preschoolers, and once a year for school aged children (5 years and older).
Two children with [REDACTED]Both children have signed Individual Health Care Plans, however, educator is missing signed medication consent forms from the parents.
A large cement step leading to the front door was loose and poses a hazard. Observed various hazards in the backyard such as metal shovels, a large pile of stick in an uncovered stone fire pit, two wheelbarrows, a lawn mower, and an old bathtub with peeling white paint.
Educator does not have separate files for her two assistants. Their current medical forms, training certificates, CPR and First aid certifications, etc. must be included. Assistant EEC certificates must also be posted next to the program license. Discussed Hannah being required to complete at least 5 hours of professional development training per year. [REDACTED] works full time with the educator and must complete 10 hours of training per year.

No current progress reports were available for review. Educator uses CDC developmental checklists and a YMCA goals sheet. Evaluations must be completed every three months for infants and children with special needs, every six months for toddlers and preschoolers, and once a year for school aged children (5 years and older).
Observed screwdrivers and dog feces on the ground in the backyard. Area with an old bathtub, a metal ladder and wood pieces/planks accessible. Discussed fencing off the space with educator. Also observed a lawn mower, a metal shovel, a metal ladder stabilizer, and an uncovered trash bin accessible in the backyard play space.
