Three children's records were randomly selected for review, and the following was learned: 1) One Child did not have evidence of a current Physical.
It was observed in the Fish Classroom that multiple rest mats were ripping, and foam was exposed.
During the safety inspection of the program, the following was found: 1) In the Fish Classroom, the dramatic play equipment needed to be secured. 2) In the Crickets Classroom, the bookcase needed to be secure. 3) Throughout the program peeling and chipping paint was observed. 4) In the Toddler Bathroom, multiple floor tiles were cracking and needed to be replaced.
During the safety inspection of the program's building and playground, the following was found: Paint was peeling or flaking off the building in multiple areas.
During the IHCP review, the following was found: A child's IHCP expired on 12/3/22 in the Crickets Classroom.
