At the time of the visit, two educators' bags were found on the floor in an unlocked closet to which the children had access.

The Caterpillar Classroom did not maintain accurate attendance. There were four children present, and three children signed in.
During the health and safety check of the program, the following was found: 1) Multiple shelves in the Caterpillar, Giraffes, and Eagle Classrooms were not secure. 2) Chipping paint was found in the Giraffe Classroom. 3) Chipping paint was found on all window sills in the Caterpillar, Giraffes, and Eagle Classrooms.
At the time of the visit, an Educator in Gross Motor wiped a child's nose and did not wash their hands.
Playground free from hazards; fenced if applicable
Eight staff records were selected for review, and the following was learned: 1) Six staff members did not have evidence of a hire date. 2) One staff member did not have evidence of two [REDACTED]as required by regulation. 3) Eight staff members did not have evidence of bimonthly observations as required.
During the visit, the kitchen door was left open and unlocked.
One of the Educators did not complete the two-step cleaning requirements in the Caterpillar and the Gazelle Classroom.

Individual Health Care Plan (IHCP) - R Apr 2025
A Trampoline was found in the Gazelle Classroom.
During the program's indoor safety inspection, the following was found: 1) The dramatic play equipment and shelves were not secured in multiple classrooms. 2) In multiple classrooms the corner of rugs were rolling up causing a tripping hazard.
During the safety inspection of the program's playground, the following was found: 1) Rust was found on both playground climbers in multiple spots. 2) The Preschool/ Toddler playground did not have adequate depth of impact-absorbing material (Mulch). 3) The Preschool's wood garden beds was found broken with screws exposed and wood spitting, causing a potential hazard. 4) The Climber material was found spitting and peeling on the stairs and the slide. 4) The gate to the Toddler Playground was found bent and warped.
In the Gazelle and Eagle Classroom, the plan for diapering and toileting training was not posted.
During the review of the fire drill log, it was found that the program did not complete a drill in January 2024.
During the IHCP and Medication review, the following was found: 1) A child's IHCP in the Caterpillar Room was expired. 2) A child's required medication (antihistamine) was not found in the emergency bag in the Giraffe Classroom.
This change of space visit inspection recorded no violations or advisories.
The emergency/evacuation posting was not posted in the Giraffe Room at the time of the visit.

At the time of the visit, in the Panda Room it was found: 1) The program did not obtain one child's medication from the parents required on their IHCP. 2) One Child's medical consent forms expired on 10/27/22.

Four staff records were randomly selected for review, and the following was learned: 1) 3 staff did not have evidence of documentation of the interview date. 2) 2 staff did not have evidence of the date of hire. 3) 1 staff member did not have evidence of Physical and [REDACTED] as required by regulation. 4) 4 staff did not have evidence of bimonthly observations as required. 5) 2 staff did not have evidence of the completion of program orientation.
During the visit, the following was observed while reviewing all indoor licensed spaces: 1) Chipping and peeling paint was found in multiple classrooms and common areas. 2) Multiple shelves and dramatic play equipment needed to be secured.
At the time of the visit, the children had access to the teacher closet. The closet door was left open and unlocked.

During the time of the visit, the water temperature in the children's bathroom was too hot to touch. The Director shared that the issue is currently being addressed.

Review of 3 children’s files found: [REDACTED]enrollment/face sheet and transportation plan were not signed or dated and [REDACTED] file did not have consent on file for the [REDACTED] to work with the child in the classroom. Progress reports for children with documented special needs have not been completed every 3 months.
There were six children in the toddler classroom and educators had not marked any children in on the attendance roster.
