At the time of the visit, the school-age group did not maintain accurate staff-to-child ratios after two educators left the blue classroom, leaving the remaining educator with 28 school-age children.

At the time of the visit, the following was found: 1) The Kitchen in the toddler classroom was not secured. 2) A plunger was found in the gym bathroom within the reach of children.

During the safety inspection of the program, the following was found: 1) The baseboard in the left Preschool Bathroom was peeling off the wall, exposing drywall. 2) Dramatic play equipment needed to be secured.

During the health and safety inspection, the following was found: 1) An outlet cover was missing in the Preschool classroom. 2) Diaper creams were found on the changing table in the Infant Classroom within reach of children.

During the program's safety inspection, the following was found: 1) Chipping/peeling paint was found in multiple areas throughout the program. 2) The dramatic play kitchen was not secured in the Back Toddler Classroom.

Disinfectant solutions was placed on a shelf and not stored in a secure place out of the reach of children.
The following prescription medications were expired: Two [REDACTED] belonging to two different children were expired. One bottle of [REDACTED]belonging to a child was expired.

The fencing to the playground posed an entrapment hazard due to the unsecured bottom of the fence. The steel climbing tower was rusting.
An educator's purse was on a chair in the classroom, accessible to children.

When observing the one of the toddler classrooms, toddlers were playing inside the sensory table with small beans and dried peas which are considered a choking hazard for toddlers. Ceiling tiles in the bathroom were observed to be stained.
