At the time of the visit, the following was observed: 1) An Educator did not wash their hands after wiping a child's nose in the Toddler Explorers Classroom. 2) An Educator did not wash their hands after wiping a child's nose in the Infant Pioneers Classroom.
At the time of the visit, the Preschool Playground did not have the adequate depth of impact-absorbing material (sand).

Three staff records were randomly selected for review, and the following was learned: 1) One Staff needed evidence of [REDACTED] as required by regulation. 2) One Staff member did not have evidence of the Date of Hire.
Three children's records were randomly selected for review, and the following was learned: 1) One children did not have evidence of yearly lead screening.
During the safety inspection of the program, the following were found: 1) Chipping paint was found in multiple areas of the program. 2) The baseboard was missing in the corner of the Infant classroom.
Developmentally Appropriate Curriculum and Activities
During observations of daily activities, it was observed that an Educator was not conducting a developmentally appropriate circle time for the ages of the children in the Toddler classroom.
During the Safe Sleep review in the Infant Classroom, the following was found: 1) Three children's crib mattresses did not have a clean covering. 2) Three sound machines were found attached/ hanging on the cribs. 3) A swaddle sleep sack not hands-free was found in a crib.

After changing a child's diaper in the Infant/Toddler Classroom, the following was observed: 1) The Educator did not wash the child's hands with liquid soap and water. 2) The educator did not complete the two-step cleaning process on the diaper changing table after the diaper change was completed.
During the staff records review, the following was found: 1) Two staff members did not have evidence of the interview date. 2) Three staff members did not have evidence of verification of 2 references. 3) One staff member's PQ Registry was expired.
During the safety inspection, the following was observed: 1) There was chipping and piling paint above the baseboards throughout the program. 2) The diaper mat and the foam climber in the Infant Classroom were peeling and ripping. 3) The heater end cap paint was peeling. 4) Multiple Shelves and Dramatic Play equipment need to be secured throughout the program. 4) Peeling/chipping paint and holes in the drywall were observed over both sinks in the preschool bathroom. 5) In the Toddler Classroom, the rug was a tripping hazard. 6) In the Preschool Classroom, a pillow/mat was stained.
At the time of the visit, written emergency/ evacuation procedures were not posted to each exit in all classrooms.
In the Infant Classrooms it was learned the sink is not being cleaned and disinfected in-between bottle warming/food prep and handwashing.
At the time of the visit, Lysol spray, hand sanitizer, and bleach solution were found within children's reach in both preschool classrooms.
At the time of the visit, not all of the children's water bottles were labeled.
At the time of the visit, an infant napping had a pacifier clip attached to thier shirt.
At the visit, 3 of the 5 Children files reviewed did not have evidence of a current progress report.

At the time of the visit, two of the four staff files did not have evidence of an EEC Essentials Certificate.
At the time of the visit, both preschool classrooms did not maintain accurate attendance. The pre-k classroom had 14 children signed in, and there were 13 children present. The preschool classroom had 7 children signed in and 8 attending.

In the Preschool Room, an Educator's bag of personal belongings including medications, was on the counter within children’s reach. There were snack items mixed in with art supplies. Disinfecting sprays and cleaners were within children’s reach and not stored away safely. Educator’s coffee cups were within children’s reach.

An educator in the infant room did not respond to a child crying in their crib in a timely matter.
During the visit, cabinets that had child proof safety locks were not secured tightly to prevent children from opening. Cabinets throughout the program were not safely secured allowing children access to staff's personal items.
During the visit, a table activity in the toddler classroom was observed to be developmentally inappropriate for the age group.
During lunchtime in a toddler room, a child was alone at a table with their lunch and not attended to for an extended time, while one educator was cleaning and the other was diaper changing.

During the visit, a broom and dustpan were accessible to children in the preschool classroom. Immediately following the visit, the [REDACTED] corrected the non-compliance by applying hooks to the wall to ensure the dustpan and broom are secured.

The daily attendance was inaccurate in the toddler I classroom and in the preschool classroom.
During this visit, Educators assigned to the Toddler II classroom were actively wiping down tables and not interacting with the children. [REDACTED]was in the classroom working one-on-one with a child and the other children were seeking her attention.

During the visit, Licensing Specialists were informed by infant classroom educator that the following was their bottle warming procedure: The educator will leave the classroom to go to the food prepping sink in the gross motor area to warm the bottle in a vat of tap water. Licensing Specialist later learned from [REDACTED] that a container of water is heated in a microwave. Then the infant's bottle of milk/formula is placed in the vat of water for warming on the counter in the infant classroom.
Opportunities to foster independence and self help skills
The pre-k and toddler classroom did not have a sufficient amount and variety of toys and materials. The pre-k classroom had empty book shelves.
Of the files randomly reviewed, one child's file did not have a [REDACTED] to take place at the program.
Through review of staff files the following was noted: One staff record was incomplete with an expired PQ (exp 10/15) and no current first aid on file.
In the Toddler II classroom, there was a dramatic play stage and kitchen set that was not secured. Cabinets with educators' belongings were not secured allowing toddlers to have access.
The outdoor space had a tent within the fall zone of the playground structure.
There was [REDACTED]found without a [REDACTED]. A [REDACTED] was ripped off the box.
Through review of [REDACTED] the following was noted: 1) One form did not have appropriate consents for medications. Medications listed were on the same form and only state [REDACTED] 2) One form did not have a [REDACTED]and did not have consent for a[REDACTED] 3) One form did not have a parent's signature for [REDACTED].
