Supervision of Children
Two Staff Records were selected for review, and the following was found: 1) One staff member did not have evidence of a current First Aid Certificate.
During the visit, Educators in Classroom 115 did not exercise appropriate supervision of the children in their care when a child was able to leave the classroom, walk to the restroom, and return without the educators' awareness.

During the health and safety inspection, the following was observed: 1) Two shelves in Classroom 116 were not secured. 2) In the Ladies' bathroom, the baseboards were peeling off the walls.
During the IHCP/Medication review, the following was found: In Classroom 114, a child's medication was not in the containers in which it was originally dispensed and with its original labels.
At the time of the visit, one educator's bag was found on the floor in an unlocked closet accessible to the children in Classroom 116.
During the program's indoor safety inspection, the following was found: The bathroom baseboards were peeling on the walls.

During the visit, the kitchen door was left open and unlocked.

At the time of the visit, both preschool classrooms did not maintain accurate attendance. The Small Preschool classroom had 9 children signed in, and there were 11 children present. The Large Preschool classroom had 12 children signed in and 14 children present.
Three children's records were randomly selected for review, and the following was found: 1) One child did not have evidence of a current physical and date of admission on record as required by regulation.
At the time of the visit, the Pre-k and the Preschool (Small) classroom's second exit/evacuation route needed to be clear of obstructions.
