A review of staff BRCs indicated that two were expired. The Program Director stated that they've been in contact with the BRC Unit for technical issues in accessing the BRC database.
It was observed in PRESCHOOL that an outlet was uncovered.
It was observed OUTDOORS in the far right area that there were large pieces of wood chips that were a potential cause for injury. The Program Director is aware and is working to resolve. Educators stated that they will try to remove the large pieces while their group is outside.
A review of five IHCPs indicated that three did not identify which staff were designated and trained to administer medication and one did not identify the potential consequences of not administering medication.
The following was observed and accessible to children: In the TODDLER/PRESCHOOL classroom, there was an uncovered outlet, a gap between the baseboard protective heat covers, and there were two uncovered holes in the half wall.
It was observed in the INFANT/TODDLER classroom that a child's medical information was not posted in a confidential manner to protect their privacy.

It was observed on the INFANT/TODDLER outdoor area that there was a rubber garden hose not secured or stored away. This was immediately removed during the visit.
It was observed in the INFANT/TODDLER classroom's refrigerator that there were two milk cups not labeled with the child's name.
A review of one IHCP and accompanying medication indicated that a secondary medication identified on the IHCP was not located onsite. The Program Director stated that they were in communication with the child's parent to provide the medication to the program.
It was observed in the PRESCHOOL classroom that an open box of disposable gloves was accessible to children.
