The playground does not have an adequate depth of safety surfacing around the swings and yellow climbing structure.
Upon a review of three Staff Records during the visit, the following was identified: -Two educator files did not have evidence of a current physical, MMR immunity, and evidence of EEC Essentials training via Strong Start.

The program did not have evidence of a current Building and Fire Inspection certificate. During the visit, the Director stated the inspector has come and the program is awaiting the certificate to be issued.

A review of three Children's Records indicated the following: -One child's records did not have a current and complete Individual Health Care Plan (IHCP) for a diagnosed allergy. -A second child's record did not have a current and complete Individual Health Care Plan (IHCP's) for a diagnosed allergy or an IHCP for an (REDACTED)diagnosis.
A review of three Staff Records indicated the following: -One educator did not have evidence of current first aid certification. During the visit, the Licensee stated that there has been limited offerings for First Aid classes on the island.
A review of three Children's Records indicated the following: -One child's record did not have a current and complete Medication Consent Form for a prescribed(REDACTED) and/or another Medication Consent Form for (REDACTED) for a child diagnosed with allergies. -A second child's record did not have a current and complete Individual Medication Consent Form for prescribed (REDACTED)and/or another Medication Consent Form for a prescribed (REDACTED)and allergies. This means in the event of a potential medical emergency, the program does not have parental consent to administer four medications to two children.

Medication
A review of the BRC Navigator indicated that the program has three out of five employees that are past due for their BRC renewal.
The playground wooden fence has several areas of rotted wood that are in need of repair.
Out of three Children's Records reviewed: -Two records lacked a complete IHCP (Individual Health Care Plan) for two individual children diagnosed with (REDACTED), as there was not an educator identified as having been trained in the administration of the child's prescribed medication nor was the person who trained the educator identified. -One record lacked a current and complete IHCP (Individual Health Care Plan) for a child diagnosed with (REDACTED) as there was not an educator identified as having been trained in the administration of the child's prescribed medication nor was the person who trained the educator identified.

Out of three Children's Records reviewed: -One record was missing a current Medication Consent Form for a child's prescribed medication for (REDACTED). The last Medication Consent Form on file was signed and dated on (REDACTED). -A second record was lacking a Medication Consent Form for Benadryl for a child with a (REDACTED).
During a review of three Children's Records, the following was identified: -One child's record did not include a contact phone number for a parent/guardian, a current physical examination or evidence of immunizations. The program did have written evidence of requesting all of this information from the parent. -One child's record did not contain evidence of a current physical, immunizations, lead screening, developmental history, first aid and emergency consent form, or transportation plan. -Children's records must have all required applicable documentation according to the Children's Record Checklist.

Medication
Upon a review of three Staff Records at the program, the following was indicated: -One staff record did not include all required EEC forms required under 7.04(18)(c) such as EEC orientation, evidence of MMR, physical exam, references, and staff information form.
The program did not have evidence of an emergency evacuation/fire drill for the month of February 2022.
A review of medications stored at the program indicated the following: -A child's prescribed (REDACTED). The medication was not stored in its original container with affixed prescription label.
