Current consent for administered medications
1. Documentation of a physical examination was not available for review in a staff’s record file. 2. Documentation of a program orientation and verification of two reference were available for review in two staff record files.
At the time of the visit, the program's Building Inspection reflected expiration date of 4/11/2025.
A staff’s personal belonging (lunch tote) was hung on a hook on the wall inside a classroom, within reach of {REDACTED} children.
1. A child’s prescription medication (Epi Pen & Cetirizine) was found to be expired as of 6/2025. An affixed label was not attached to the original packaging/box of the Epi Pen medication. 2. An over-the-counter medication (Zyrtec) for a child diagnosed with a chronic medical condition was not available for review.
At the time of the visit, a child’s individual health care plan was outdated as of 1/17/2025 and the IHCP did not contain a signature from the parent.
First Aid certification was not available for review for a staff member hired on 1/8/2024.

Eight sunscreen bottles were stored inside a caddy and placed on top of a small shelf which at the time of the visit was found to be within reach of children.
The staff meetings reviewed did not include documentation of at least two hours per month for meetings conducted in January, March, May and July of 2022.
