1. The outdoor storage shed reflected three small screws protruding from a wood panel that is detaching from the door. 2. The program's playground outdoor fence reflected a gap opening, approximately 10 inches wide located on the lower end of the fence which may present a safety hazard.

At the time of the visit, a qualified administrator was not on site. Staff were not able to identify the designated staff in charge.
1. A children’s records checklist was not available for review. 2. Four children’s records reviewed did not contain documentation of an updated developmental history form. 3. Three children’s records did not contain documentation of an updated physical examination. 4. A child’s record did not contain documentation of a lead screening and an updated immunization record as of 2021.

The Building Inspection posted on site and submitted to EEC expired 6/21/2023. The Fire Inspection posted on site and submitted to EEC was valid until 5/4/2023.
1. The outdoor chain link fencing exposes introverted wiring located on the bottom of the gate. 2. Small gaps between the ground surfacing and the bottom edges of the chain link fencing were observed, which may lead to a child entrapment hazard.
At the time of the visit, the January 2023 through September 2023 emergency evacuation observations (practice fire drills) were not available for review.

A review of three staff records reflected the following. 1. Documentation of bi-monthly observations were not available for review for three staff. 2. A staff's record did not reflect dates of {REDACTED} immunizations. A second staff record did not reflect a date for their second {REDACTED} immunization. 3. Documentation of an annual evaluation was not available for review for one staff member. 4. Documentation of First Aid Certification was not available was not obtained for a staff member working past the six months of employment.
At the time of the visit, sufficient materials were not accessible to children in a toddler classroom as toy shelves were turned around and placed against the wall which prevented children from accessing materials.
Evacuation procedures were not posted at two exits in a {REDACTED} classroom.
At the time of the visit, a child's individual health care plan did not include parental consent. An Individual Health Care Plan was not available for review for a child requiring the use of an {REDACTED} medication. A child's doctor's order for an {REDACTED} medication dated 9/24/2021 (valid until 9/24/2022). A child's Medication consent form for {REDACTED} medication was outdated as of 9/27/2022.
Disinfecting/Sanitizing solutions used near the diapering area were not properly labeled.
