Toxic Substances
A doctor's medication order was kept on file in lieu of an individual health care plan for a child diagnosed with a chronic medical condition which did not contain written parental consent. Children diagnosed with a chronic medical condition, must have an individual health care plan on file.
1. Three Liquid Whiteout products and three containers of disinfecting wipes, indicating "Keep Out of Reach of Children" were stored on top of a shelf, within reach of children. 2. Plastic trash bags were stored inside an unlocked counter drawer, accessible to children. 3. Adult size scissors were stored on top of a shelf, within reach of children.
Two staff records did not contain documentation of bimonthly staff observations.

Documentation of a January 2022 and April 2022 staff meeting was not available for review at the time of the visit. Documentation of staff meetings did not include the time of when each staff meeting was conducted for meetings occurring FY 2022.
At the time of the visit, the allergy list was not privacy protected in a preschool classroom. A child [REDACTED] was not documented on the allergy list.
