All of the children's records were reviewed. Three children did not have lead testing results, one child did not have their updated immunizations, three children's general permissions were not fully completed. Two children did not have a medication consent for a life saving medication or an individual health care plan for a medical condition.
Provider is not documenting the number of children on the evacuation log and not accurately documenting the floor level.
The provider did not have her assistant's information in her parent handbook.
Provider was observed to change the children’s diapers and did not wash the children’s hands or her own hands. The children were observed to have lunch without washing their hands.
