The heater in preschool 2 was loose. The pipe was loose causing the heater to move, it needs to be secured. There was also another piece of the heater near the teachers desk that was loose and could be a risk to children.

An inhaler was expired for one child and an inhaler for another child was not stored in the original container with the label affixed to the box.

It was observed that the outdoor space contained hazards, including overflowing debris/trash in an uncovered garbage can, tires and tree stumps located on the blacktop surface, and an excessive amount of materials creating an unsafe environment and limiting the space available for children to play.
This change of space visit inspection recorded no violations or advisories.
It was found that an educator's pocketbook and cleaning products stored in an unlocked cabinet were accessible to children, and that a shelf and two kitchen sets were tippable and pose a hazard to children.

The program does not have a qualified administrator, and the educators were unaware of who was the designated person in charge on the day of the visit.

During a review of staff records on 12/13/2022, it was found that some of the files did not contain required documentation, including physicals, MMR's, BRC suitability letters, and some forms were missing staff signatures.

During the review of children's records on 12/13/2022, it was found that some of the files were missing updated physicals and that all files were missing the EEC transportation plan and authorization form.
During the visit, none of the staff had documentation of current First Aid training. 7.09 (17)(d) states, All educators must obtain within six months of employment, and must maintain thereafter current certification of training in basic first aid appropriate to the population served.
The program is located on the 2nd floor and windows in the children's bathroom were not locked and windows guards were not in the locked position. In addition, toxic substances were found in an unlocked cabinet under the sink in the younger preschool classroom.
On 11/30/2022, staff reported that fire drills have not been conducted and recorded on a monthly basis.
During the visit, it was found that suitable guards were missing from the windows in the girl's bathroom.
