While reviewing the Staff Records Checklist and the Staff Educator Schedule there was no evidence that all educators have First Aid.
While reviewing the Staff Records Checklist and the Staff Educator Schedule there was no evidence that at lease one staff person certified in CPR was present at any given time while children were in care.
One educator listed on the Staff Schedule did not have a current BRC, theirs expired in January 2026.
The Allergy/Special Conditions list posted in the toddler/preschool classroom was outdated. The children listed were no longer in that classroom and a child in that classroom with [redacted] and medication was not listed.

A child prescribed an Epi-pen for a peanut allergy did not have an epi-pen at the program. A child was missing Benadryl that was required for their IHCP.

The preschool teacher did not have evidence that she was a qualified preschool teacher. Two staff files were missing current physical and immunizations. Two staff files reviewed were missing First Aid and CPR certifications.
While observing the classrooms, educators were standing around the room watching the children, but not engaging with them or doing their planned curriculum activities.
One child was missing from the infant classrooms attendance.

Each room with infants has a specific evacuation route and an evacuation crib. Evacuation drills have not been documented since August 2023. It was reported they are done monthly.
The staff meeting agenda's were only available for November of 2023. The Director reported that staff meeting are being held monthly and all staff are attending, but the documentation was not made.
