Observation and inspection of the playgrounds revealed the following: 1) The fence in the Toddler Playground had an area where the fence was curled up and could pose a danger for children. 2) There was a cement footing that was exposed, in the toddler playground, at the gate leading in and out of the playground. 3) The dirt along the edge of the cement landing, at the bottom of the stairs in the toddler playground. Is issue created a step that was too high for toddler children to manage.

EEC Licensor saw an 8 month old infant sleeping in a swing on the day of the visit. The Director reported that the child had a medical condition but the program did not have a written notice from the child's physician on file for the alternate sleep position.

On the day of the visit the wood mulch in the toddler playground appeared to have pieces that could pose a choking hazard for toddler children.
